The following form is an important legal document. It explains the terms of your human design session with Danielle Allen (“Human Design Reader”). Please read it carefully and understand it fully before signing.

PROFESSIONAL BOUNDARIES + ETHICS:
Human Design Reader is not responsible for decisions or predicting the future, but rather is a healer who helps guide you to self-understanding and self-worth. Ultimately, you are responsible for your life, Human Design Reader will empower you in this knowing.

WHAT HUMAN DESIGN IS AND ISN’T & HOW HUMAN DESIGN READER OPERATES:
Human Design is a tool, much like a blueprint or DNA sequencing. They are a guide for self-exploration, awareness, and understanding which ultimately empower you to use your own free will. They are not tools for predicting the future. Human Design Reader is committed to the advancement of Human Design as a science and the development and enhancement of the human condition through a deep understanding of celestial phenomena. Your chart reading may touch upon personal and professional subjects in your life. However, your reading should not be used in place of an attorney, doctor, tax accountant, or financial adviser.

CONFIDENTIALITY:
In line with standard practice, all information provided by you during your Human Design reading will be kept in strict confidence unless disclosure is required by law. In other words, generally speaking, Human Design Reader will not disclose your name, personally identifiable information, or details of your session unless; 1. An indication of intent to harm self or others. 2. Any information regarding child abuse. 3. A court order requiring the release of information.

CANCELATION POLICY:
I understand that canceling my appointment without giving at least 48-hours’ notice will result in the loss of my session fee. Should I decide to cancel my appointment (prior to the 48-hour mark), I acknowledge that Human Design Reader will take up to a week to return monies.

NO-SHOW POLICY:
I understand that if I do not show within the first 15 minutes of my scheduled session, the session counts as a no-show and is subject to the cancelation policy. This results in the loss of my session fee.

REFUSAL OF SERVICE POLICY:
I acknowledge that Human Design Reader has the right to refuse service based on her discretion, but also for health, safety, or other similar reasons, such as me being unruly, disrupting Human Design Reader's business operations, or causing injury.

ACKNOWLEDGMENTS:
I acknowledge and agree that I voluntarily engage Human Design Reader to provide a Human Design session.

I acknowledge and agree that Human Design Reader is someone who offers a perspective or opinion based on a thorough analysis of my Human Design chart with info taken from my birth data.

I acknowledge and agree that during my session, Human Design Reader will rely on information provided by me and I affirm that all information provided to Human Design Reader is true and accurate to the best of my knowledge and belief.

I acknowledge and agree that Human Design session should never be used in place of professional counseling.

I acknowledge and agree that this consultation does not constitute legal, medical, or financial advice.

I acknowledge and agree that Human Design Reader makes no guarantees, representations, or warranties of any kind or nature, express or implied with respect to the consultation services negotiated, agreed upon, and rendered. In no event shall Human Design Reader be liable to me for any indirect, consequential, or special damages. I release and fully discharge Human Design Reader of and from any and all claims, demands, causes of action, and rights, whether known or unknown, relating in any way to my dealings with Human Design Reader.
